About Us
Life Care Hospice staff and volunteers unite with the community to accomplish a single goal: To provide the patients in our Hospice Program and their families the highest quality of care possible so they may life each day to the fullest. Hospice provides a variety of services tailored to meet the individualized needs of patients and their family members. Services are available 24 hours a day, 7 days a week. Every person is unique and hospice care is no different. Each patient and family’s plan of care is unique . Hospice strives to meet the immediate needs of the patient and family, while making every effort to anticipate and meet future needs.
Additional Information
You are eligible for hospice when your doctor and the hospice medical director certify you have a life-limiting illness have a prognosis of six months or less if the illness runs its normal course, and you agree to receive palliative care verses curative therapy. Our mission at Life Care Hospice is to provide a higher quality of care for all patients and their families when confronted with a life-limiting terminal illness. We recognize that death is a normal process in life and we strive for excellence in compassionate medical care. Our staff will serve as a voice for the terminally ill to help them live their final days with dignity while coping with grief and the end-of-life. Hospice affirms life and focuses on the quality of life. Hospice exists to provide support and care for persons in the last phases of an incurable disease so that they might live as fully and comfortably as possible.
